Level 1
This level of service is for individuals that need limited assistance with activities of daily living.  Services and care includes
  • Use of common areas and choice of available suits/rooms
  • All utilities including basic cable and common telephone
  • Individualized care plan
  • 45 day wellness assessment by licensed registered nurse
  • Assistance with prescribed medication by certified medication technicians
  • 24 hours assistance call available
  • Every 4 hours safety check
  • Three nutritious, delicious meals a day including snacks
  • Limited physical assistance with dressing, grooming
  • Limited assistance with bathing (up to 3 times a week)
  • Self managed incontinence reminders
  • Daily trash removal and spot cleaning
  • Weekly housekeeping and laundry service
  • Scheduled transportation for shopping, doctor's appointment
Social, recreational, educational, religious services and activities
 
Level II
This level of service is for individuals that need moderate assistance with activities of daily living.  Includes all level I services plus
  • Moderate physical assistance with dressing and grooming daily
  • Increased safety checks as needed
  • Physical assistance and management of occasional incontinence
  • Assistance with meals set-up
 
Level III
 
    This level of service is for individuals that need heavy assistance with activities of daily living.  This includes level I & level II services plus
     
    • Regular assistance with dressing and grooming
    • Assistance with feeding
    • Daily assistance with bathing
    • Regular incontinence care
    • Additional laundry and housekeeping service
    • Transfer/mobility assistance
